Referrals and forms

All referrals should be sent using the Electronic Referral System (ERS) using the standard MSK Sheffield Referral Proforma. 

Please choose ‘Pain – Pain Management – MSK Sheffield’ on ERS:

All referrals will be triaged by a Consultant to direct the patient to the right MSK Sheffield service.
